Over 1,000 GP practices make referrals to Social Prescribing Link Workers daily. The data generated is providing ICSs with the population health data that they need to make better informed commissioning decisions.
As part of the enhanced integration Social Prescribing Link Workers now use a series of automated drop downs (and SNOMED Codes) within Elemental when processing referrals which means they don’t have to enter data into Elemental and into your clinical system – this saves them time!
Additional case notes made visible within the clinical system means for the first time GPs will have full oversight of how a patient is engaging in social prescribing and how their health and wellbeing is progressing. Creating an even more joined up conversation between patient, link worker and the GP.
