Mosaic has helped social workers simplify the way they record case notes, enabling them to better manage their community’s social care needs, bringing them closer together for Digital Care.
How we helped
Mosaic, the single-platform case management solution across social care, has been deployed by Cambridgeshire County Council to support the 800-strong adult services team. Frontline staff, including social workers, now have a clearer digital picture of the care pathways available to those in need and faster, more secure communication, using the new collaborative platform.
We’re calling Mosaic our ‘system for the system’. From procurement to migration and implementation, they have worked in partnership with us, and the completion of the first phase is something I, as project sponsor, am really proud of.
The results
- As well as encouraging the adult services team to share best practice, Mosaic has helped the social work team record better case notes, with clearer pathways to better care for service users.
- Frontline staff now have a clearer digital picture of the care pathways available to those in need.
- Mosaic has enabled social workers and outreach teams to integrate forms and reference the reports and data which are required to meet statutory and legislative requirements.
- The new system is consistent and fit to support Cambridgeshire County Council’s digital ambition.
Inspiring others
From procurement to migration and implementation, they worked in partnership with Cambridgeshire County Council. Mosaic is really making a difference to Cambridgeshire’s frontline workers. They are inspired and excited by the breadth of the capabilities of the new system and the time it’s saving them. Through Mosaic, the Council is already benefitting from improved access to core, real-time data. The new system offers the opportunity to make links between social care data and summary health records. This interoperability between key systems is something Cambridgeshire County Council is looking to develop with its partners and IT system suppliers in the future.
